  • Collective PT is an Out of Network (OON) provider. We accept credit/debit/HSA/FSA cards for payment at time of service. If you have out of network benefits, we can provide you with a superbill to submit to your insurance for reimbursement. Please check with your insurance company for your out of network benefit coverage.

  • Since Collective PT is an out of network provider, we can not provide services or care to patients who utilize Medicare for their insurance coverage. We can not submit claims to Medicare for your care. This is a Medicare mandate.

  • No. Illinois is a direct access state meaning that you do not need to see a physician prior to attending physical therapy.

  • A pelvic floor assessment can include an external/internal vaginal or internal rectal assessment to assess the connective tissue, muscles and ligaments to help decrease your symptoms.

  • Please wear clothes that you feel you can move around in. If you have specific questions for the body part you are being treated please email prior to your initial evaluation.

  • Each session is 50 minutes long.

  • Your child is welcome to attend, but there is not additional child care staff.
